To be completed by all prospective clients of the Sweep 'n Suds. Any and all applications may be rejected at the discretion of management.
The submission of the above-application in no way binds applicant and Sweep 'n Suds or its employees until signed by all parties. A fully-executed copy of such will be provided to applicant upon request at no charge.
